The Health and Wellbeing Visits Project

The project aims to reduce health inequalities and increase life expectancy for vulnerable older people living in deprived neighbourhoods. Fully trained Community Volunteers will visit people in their own homes to talk about their health and wellbeing; providing support to assess and understand their own health needs and raising awareness about the choices available to them.



  • people over 50 with disabilities or long term conditions
  • over 65 who
    • ​face social exclusion
    • live in communities or groups facing health inequalities
    • live alone
    • are income deprived
  • ​people over 80
